Department of Mines and Petroleum figures reveal safety inspectors are handing out 140 "prohibition notices" forcing companies to shut down their mines or equipment when there is "imminent danger" to workers every year.
They are also handing out 13 "improvement notices" a week for serious second-tier safety issues. With 350 active mine sites, it means every mine in WA is being censured an average of twice a year for putting workers at risk.
The Sunday Times has also learnt the department is probing four serious safety breaches at Newmont's Boddington Gold Mine in recent months, including a worker crushed by a one-tonne metal plate in a mill who survived only after being resuscitated.
Other breaches include a cyanide truck failing to stop at an intersection on a road used by the local school bus; a truck rollover; and a loaded crane that rolled over
